ACP Billing Guide
Billing Threshold
CMS allows billing 99497 at 16+ minutes (not full 30). Each additional 30-min block adds 99498 ($75).
AWV Add-on (Copay Waived)
When ACP is billed same day as AWV (G0438/G0439) with modifier -33, the patient's copay is waived. Check "Billed with AWV" when creating records.
Documentation Required
Must document: who was present, topics discussed, and patient decisions. Voluntary for patients.
